Osteomielitis por Salmonella en un paciente pediátrico con fractura de húmero proximal

  • Una niña de 5 años previamente sana sufrió una fractura de húmero proximal izquierdo después de una caída desde un trampolín. Inicialmente, la lesión se trató sin cirugía, pero durante el seguimiento de rutina, se descubrió que el paciente tenía una erosión ósea no reconocida previamente en el sitio de la fractura. El estudio fue negativo para neoplasia, pero en cambio demostró osteomielitis por Salmonella. El paciente fue sometido a desbridamiento y tratamiento con antibióticos.
  • Esta es una presentación inusual de una fractura de húmero en un niño con osteomielitis por Salmonella concomitante. La osteomielitis es una consideración diagnóstica importante cuando se consideran erosiones óseas en el paciente pediátrico, incluso en aquellos pacientes sin factores de riesgo aparentes.

Abstract

OBJECTIVES:

The current literature focuses on wound severity, time to debridement and antibiotic administration with respect to risk of infection after open fracture. The purpose of this analysis was to determine if either the incidence of post-traumatic infection or causative organism varies with treating institution or the season in which the open fracture occurred.

DESIGN:

Retrospective review.

SETTING:

Seven level-one regional referral trauma centers located in each of the seven climatic regions of the continental United States (Northwest; High Plains; Midwest/Ohio Valley; New England/Mid-Atlantic; Southeast; South; and Southwest).

PATIENTS/PARTICIPANTS:

5127 skeletally mature patients with open extremity fractures treated between 2008 and 2012 at one of the seven institutions.

INTERVENTION:

Open reduction and internal fixation of fracture following institutional protocol for antibiotic prophylaxis, debridement and soft tissue management.

CONCLUSIONS:

A substantial seasonal and institutional variation exists regarding the incidence of infection and causative organisms for post-traumatic wound infection following open fractures. While this may represent a difference in treatment regimens between individual surgeons and institutions, a decades-old general nation-wide empiric antibiotic prophylaxis regimen for all open fractures may in fact be outdated and suboptimal. We recommend that surgeons consult with their infectious disease colleagues to better understand the seasonal variation of infection and causative organism for their individual hospital, and adjust their prophylactic and treatment regimens accordingly.

OBJETIVOS:
La literatura actual se centra en la gravedad de la herida, el tiempo para el desbridamiento y la administración de antibióticos con respecto al riesgo de infección después de una fractura expuesta. El propósito de este análisis fue determinar si alguno de la incidencia de infección post-traumático o microorganismo causante varía con el tratamiento de la institución o la temporada en que se produjo la fractura abierta.

AJUSTE:
El siete centros regionales un nivel de referencia de trauma ubicados en cada una de las siete regiones climáticas de la parte continental de Estados Unidos (noroeste; High Plains; Medio Oeste / Ohio Valley, Nueva Inglaterra / Mid-Atlantic; Sudeste; Sur; y Southwest).
Pacientes / Participantes:
5127 pacientes con esqueleto maduro con fracturas de la extremidad abierta tratados entre 2008 y 2012 en una de las siete instituciones.
INTERVENCIÓN:
La reducción abierta y fijación interna de la fractura siguiendo el protocolo institucional para la profilaxis con antibióticos, desbridamiento y de los
tejidos blandos.
CONCLUSIONES:
Una variación estacional e institucional sustancial existe con respecto a la incidencia de la infección y organismos causantes de infección de la herida post-traumático después de fracturas abiertas. Si bien esto puede representar una diferencia en los regímenes de tratamiento entre cirujanos e instituciones individuales, un régimen de décadas de edad, en general en todo el país la profilaxis antibiótica empírica para todas las fracturas abiertas de hecho puede estar obsoleto y sub óptimo. Recomendamos que los cirujanos consultar con sus colegas de enfermedades infecciosas para comprender mejor la variación estacional de la infección y el organismo causante de su hospital individuo, y ajustar sus regímenes profilácticos y de tratamiento en consecuencia.
